I made my own foam tire insert that I think will work quite well, but when strapped onto the rim, it seals off the end of the valve, making it difficult to inflate and deflate the tire. I see a few companies that are making valves with a little extension that allows air to pass the foam insert. What are my options for that, and do you have a favorite?

I also saw someone push a valve through a segment of plastic hose to solve that problem. Anyone else remember seeing that?

The valves that Rimpact includes with their inserts are the best I have used, period, for use with inserts or not, they appear to be the same as Cushcore sells, too. Both will sell them separate from their inserts, Rimpact sells theirs for considerably less although their location across the ocean may be an issue for shipping depending on your location.
