It's a total booger, I know. I just used a dremel cut-off disc to grind two notches in my plastic cable holder that makes threading way easier. One was through the wall against which the head of the cable sits, and another was cut obliquely through the outside wall which lines up with the direction the cable exits the red plastic disc. I thread the cable through the oblique cut and it slides easily into the exit hole. I then slide most of the cable through the shifter and then simply snap the head into its little well through the second cut, which I made pretty narrow so that the cable could not lift out easily. Super simple to replace the cable now, and doing the mod took about 15 seconds with the dremel. I attached a (grainy) pic below.
