im not sure if i have this together right, i have the derailleur hooked in through the quick release axle and the thing seems to be coming loose all the time and my wheel acually fell off once, but there are two holes the is one treaded hole that the derailleur is covering up and there is one to the right on the pic but it looks like a hex wrench would go in it. Any ideas of what the problem might be? Thanks.

the threaded hole is where a normal [hanger-mounted] derailleur would go. The one to the left that takes a hex key is where the derailleur hangar bolts on. I'd guess the QR is just not tight enough.

uh.....
you might want to take a look at the shimano tech docs. That's not a hone or saint hub and AFIK hone derailleurs aren't compatible with any QR hub.

In your second picture it looks like the QR isn't in the correct location judging by the wear mark on the frame. Compatibility might be the problem as previously noted.
