I picked up an older Cannondale Super V-1000 (99 I think) with a dead Cane Creek AD-4 shock. I need to get a replacement. Compressed the shock measures 4 inches from eye to eye and uncompressed (fully extended) it measure 5.5 inches. So should I be looking for 4" eye to eye with a 1.5 inch throw? Thank you in advance for your help.

Extended measurement, and stroke. In this case, a 5.5x1.5 is needed.

Have you contacted Cane Creek about a seal kit? If they still have them, it's only $20 or so and a very easy DIY fix.
