I enjoy adjusting the gears on my bike. Not really.

My bike is set up with 1x SLX m7000 10spd with a SunRace MS3 11-40t cassette.

My question is, why does my bike not shift well upside down, even when indexed, compared to when it is the right side up, where it shifts perfectly well?

please keep in mind I don't have a repair stand and probably can't get one, any workarounds?

Maybe because it isn't designed to be ridden upside down? I imagine how gravity affects it is your issue. Perhaps you can rig up some type of temporary stand.

I get the opposite problem. I can tune up my der upside down and get it shifting perfectly and it goes to hell rightside up. And loaded.

Gravity applied in the opposite direction makes a difference!

I've set up 3 'eyebolts' in rafters of basement ceiling with rope to position bike at a comfortable work position. I use rope at each end of handlebars and around seatpost area. Works fine for many mntc tasks including shifting adjustments.
