I wonder if the lever body is defective. Doesn't look there is damage any where else in that area.

Yeah, I do the same. I'm beginning to wonder if my lever housing was defective or had a void in the material or something because although I keep the clamp bolts relatively loose, it did not move. It must have made just the right contact with a rock in order to break in the manner it did. Good news is that it stil works but I am able to push the lever away from the handlebar quite far an in a way not possible with a properly working lever. There is also a bit of excessive movement about the lever's pivot bolt.
