Derailleurs, which leads to issues such as chainsuck, busted deraulleurs, etc. Addressed by Rolhoff, but with added drag and weight.
Flat tires, largely addressed by tubeless tires, but UST leads to its own issues.
Suspension pivots, which need to occasional replacement, and more maintanence.
I guess further development will be the areas of gearing. I think what is holding most people back from internal gearing is cost and weight. If somebody comes out with a lighter usable internal gear hub that holds up to mountain use, it would probably finally kill the deraileur.
