Whit he wheels off the bike, spin the axle by hand. If it feels like it grinds then it needs repacking/replacing/adjusting.
If it has lots of play(movement) in it, then it also may need repacking, replacing/adjusting.
You can do it yourself, but if you feel you won't be able to, then take it to your LBS(local bike store..).
