My 2003 formula hub, branded as a cannondale hub, has 2 sealed bearings. They were about $14 each from a local bearing place. To disassemble grab the outer spacer/flanges and pull them out, Then you will have to pry the bearings out.
Get a cylindrical punch from a good hardware store and knock the bearings and end-caps out from the opposite side. The end-caps are just press-fitted in there.
