I'm looking for a stem of approx. 95mm+ in length and approx. 30mm+ rise. Or, to put it another way: A 3.5" long by approx. 1" rise stem.

Here is my current stem.

From the numbers visible in pics 1 & 3, you have a 120mm, 5 deg, 25.4 dia stem. Have you checked with a LBS or maybe Ebay, saw a few 90mm & 100mm with 20 deg rise.
