Mechanical disc brake fixed pad

On my mechanical disc brake, I can put the fixed pad in the right place, tighten the lock screw until it feels like my hex key might snap, but after no more than 3 mins riding the fixed pad has moved out and it takes a lot more pressure on the brake to do anything.
